COVID-19 Release

Prior to the start of my service, I confirm that:

  • I have not been diagnosed with or cared for someone diagnosed with COVID-19 in the past two weeks.

  • I have not shown symptoms of COVID-19 or come in close contact with anyone exhibiting these symptoms in the past two weeks.

  • I have not traveled outside of my immediate daily routine for the past two weeks.

  • I do not have a cough, fever, chills, shortness of breath, or loss of taste or smell.

  • If I begin to show symptoms of COVID-19 within the next two weeks, I will contact my stylist.

  • I will follow all posted salon rules to keep myself, my stylist and those around me safe.

ASSUMPTION OF RISK AND WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19

Due to the outbreak of the novel coronavirus (COVID-19), Honey Salon is doing everything we can to protect you, our clients, our community and our staff. Honey Salon has put in place preventative measures to reduce the spread of COVID-19 by following the Center of Disease-Control (CDC) and Washington State guidelines with regard to social distancing practices and sanitation. However, Honey Salon despite its best efforts, cannot guarantee that you will not become infected with COVID-19.

Therefore, as a client seeking to obtain services from Honey Salon, I acknowledge the contagious nature of COVID-19 and by accepting an appointment at Honey Salon, I voluntarily assume the risk that I may be exposed to or infected by COVID-19 and that such exposure may result in personal injury, illness, permanent disability, and death. I understand the risk of becoming exposed to or infected by COVID-19 at Honey Salon may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Honey Salon, employees, volunteers and other clientele.

By accepting an appointment at Honey Salon, I hereby release and agree to hold Ponytail, LLC, dba Honey Salon, its members, officers, directors, employees, contractors and agents (collectively “Honey Salon”), harmless from and waive on behalf of myself, my heirs, agents, assigns and any personal representatives, any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damages or loss to myself and/or property that may be caused by any act, or failure to act of the Honey Salon, or that my otherwise arise in any way in connection with any services received from the Honey Salon. I agree to release Ponytail, LLC, dba the Honey Salon, its members, officers, directors, employees, contractors and agents, from any and all liability for the unintentional exposure or harm due to COVID-19.
